The patient portal is available to allow patients to add their medical history prior to their initial appointment and to communicate with us via the internet. A valid e-mail address and accurate demographic information is required for us to invite you to access the system.
Entering your medical history prior to your visit streamlines the process and can avoid delays in being seen by the doctor. You may still be asked to review this information with a staff member prior to seeing the doctor. If you have questions about the process, please call our main number and one of our staff members will be happy to assist you.
NOTE: We are not alerted when these messages are sent so for communication that requires immediate response, calling the office is still the best way to reach us.
This portal is available only after providing initial demographic and insurance information to our staff at the time of scheduling your first appointment. If you have not seen us since we began using our current electronic medical record system in 2011, you may be asked to enter the information prior to your next visit.

Account registration requires that the information that the information you enters matches exactly what we have in our system. If you have difficulty in registering a new account, please call the office to ensure that we have the exact spelling of your name and date of birth.
At each visit, you may still be asked to fill a brief form called a Review of Systems. This helps us identify other changes in your health that are relevant but may be overlooked in the course of a focused visit.
